The examples below are intended to help clarify common types of government and benefits fraud, but they are not limited to the scenarios listed below. If you are unsure which form to complete, select the option that best matches your concern. Your submission will be reviewed and routed appropriately if needed.

Benefits Fraud
  • Billing for services not actually provided
  • Falsifying documentation to receive a benefit
  • Using stolen identities to file claims
  • Kickbacks or improper incentives
Government Fraud
  • A public employee stealing cash, checks, or electronic payments
  • Submitting false invoices to a public entity
  • False reimbursement claims
  • Using public staff, vehicles, or equipment for personal or political purposes
